Moore, who has appeared alarmingly thin since her breakup with cheating husband Ashton Kutcher, is being treated for eating issues and is planning to enter rehab for substance abuse, sources say. “She has not taken care of her health at all lately and has lost a ton of weight,” says a friend. The actress’s spokeswoman said she was seeking help handling stress and lack of sleep. “Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health,’’ the spokeswoman said.
